InkedUp Tattoo Consent Form

I, the undersigned, confirm that I am 18 years of age or older and consent to receive a tattoo from the artist named on this booking.

I understand that tattooing involves the use of needles to deposit pigment into the skin, and that the procedure carries certain risks including but not limited to: infection, allergic reaction, scarring, and fading over time.

I confirm that I am not under the influence of alcohol or drugs. I confirm that I do not have any medical conditions that would prevent me from safely receiving a tattoo, or have disclosed any relevant conditions above.

I understand that the deposit paid is non-refundable if I cancel with less than 48 hours notice. I agree to follow all aftercare instructions provided by the artist.

I release the artist and InkedUp from liability for complications resulting from failure to follow aftercare instructions or from pre-existing medical conditions not disclosed at the time of booking.
